Sample Filing Timeline

Once listed, a PSO must timely submit periodic certifications and other information required by the Patient Safety Rule. The PSO Profile Form is not required but submission is encouraged. The timeline below has two sections: "Scheduled" and "As needed."

  • Scheduled: Displays recurring deadlines that are fixed as well as those calculated based on the individual PSO's initial listing date. Includes both required and voluntary submissions.
  • As needed: Describes the submission timeline for forms that become required only if and when certain circumstances arise.
 

Year 1: 

Scheduled:

  • PSO Profile Form
    Submit by February 28th every year. Frequency yearly; quarterly updates as needed. Voluntary submission.

As Needed:

  • Disclosure Form
    Submit within 45 days whenever the PSO has relationships with a provider that fit the description in section 3.102(d)(2)(i) of the Patient Safety Rule. Frequency: As needed. If/when applicable, required to maintain the listing.
  • Change of Information Form
    Submit whenever any information contained in the previous listing form changes. Frequency: As needed. If/when applicable, required to maintain the listing.
 

Year 2:

Scheduled:

  • PSO Profile Form
    Submit by February 28th every year. Frequency yearly; quarterly updates as needed. Voluntary submission.
  • Two Bona Fide Contracts Form
    Submit after the beginning of each 2 year period and not later than 45 days prior to the end of the period when the PSO has contracts that meet the requirement. Frequency: Every 2 Years. Required to maintain the listing.

As Needed:

  • Disclosure Form
    Submit within 45 days whenever the PSO has relationships with a provider that fit the description in section 3.102(d)(2)(i) of the Patient Safety Rule. Frequency: As needed. If/when applicable, required to maintain the listing.
  • Change of Information Form
    Submit whenever any information contained in the previous listing form changes. Frequency: As needed. If/when applicable, required to maintain the listing.
 

Year 3:

Scheduled:

  • PSO Profile Form 
    Submit by February 28th every year. Frequency yearly; quarterly updates as needed. Voluntary submission.
  • Continued Listing Form
    Submit no earlier than 6 months and no later than 75 days prior to the end of each 3-year listing period to begin the continued listing process and avoid automatic expiration at midnight of the last day of the 3-year listing period. Frequency: Every 3 Years. Required to continue the listing.

As Needed:

  • Disclosure Form
    Submit within 45 days whenever the PSO has relationships with a provider that fit the description in section 3.102(d)(2)(i) of the Patient Safety Rule. Frequency: As needed. If/when applicable, required to maintain the listing.
  • Change of Information Form
    Submit whenever any information contained in the previous listing form changes. Frequency: As needed. If/when applicable, required to maintain the listing.
 

Year 4:
